If someone warms up properly, is relaxed, and is maintaining good intonation, what would cause their voice to crack sometimes? And what can be done to help prevent voice cracking?

Hi, Kevin! Cracking occurs when we momentarily lose muscle coordination in the vocal folds - sometimes from pushing more air at the folds than they can resist, sometimes from the TA (chest voice) muscles "letting go" abruptly instead of smoothly handing off to the CT (head voice) muscles.
